If you agree to participate in a research study, you should know that none of your
legal rights are waived or affected by signing an informed consent form. If you
feel your rights have been violated or compromised, please
contact the Independent IRB, Inc.
The following are your rights as a research participant:

You have not waived any of your legal rights.

You have the right to withdraw from the study without penalty or loss of any benefits
to which you are otherwise entitled.

|
You need to notify the study doctor in writing if you want to withdraw your personal
health information from the study.
|
|
You need to request in writing to the attention of the study doctor if you wish
to withdraw specimens that were collected for future research purposes.
|
|
You are entitled to a portion of the payment for your study participation depending
on the length of your participation in the study.
|
Even after you have signed an Informed Consent Form, you have the right to withdraw
from the study at anytime.
